Carexroads is a care coordination and provider discovery platform. Carexroads is not a healthcare provider, health plan, or healthcare clearinghouse.
Carexroads operates under a HIPAA-aware platform design with PHI-minimization architecture. We do not function as a Covered Entity under HIPAA, and we are not intended to store or process protected health information (PHI).
Smart Match collects only the minimum information needed to surface eligible providers: service category, ZIP/city/county/state, funding source, urgency, preferred contact method, and the contact details you provide.
Users should not submit medical records, diagnoses, insurance identification numbers, Medicare numbers, Social Security numbers, or other protected health information through the platform. Free-text fields are reviewed and PHI-like content is filtered.
Smart Match compares providers based on services, location, funding sources, availability, and other relevant criteria, and enforces state-licensing and credential eligibility before any provider is shown. Carexroads does not make clinical decisions. Users remain free to evaluate providers independently and choose the services that best meet their needs. Every search and routing decision is audited.
